Paperwork Requirements


Once registered, the online registration system allows you to print all required forms from the HOME page.  Please be sure to download and print the following:


1.  Official AYF Player Pack - 8 pages (includes)

  • AYF American Participation Tracking and ID Card (2 page document) Signed and initialed
  • Code of Conduct Signed
  • Signed Waiver and Release of Liability
  • Concussion Acknowledgement
  • Annual Physical form OR a Medical Clearance signed by physician.  MUST be dated after January 1 of the current season!
  • Signed Emergency Medical Treatment Consent
  • Signed Image Release

2.  You will also need to submit the following (not included in the registration download)​

  • 2 copies of the FINAL Report Card (must include ALL terms of the current school year)
  • New Players - Original Birth Certificate & Copy of Birth Certificate (You will be given back the original once certified, copy will be kept)
  • Returning players from last season do not need to bring a birth certificate 



3.  Absentee Form​ (required for CHEER ONLY, do not date)


​There will be a Paperwork/Report Card Collection night in June.
